If you are a professional working in a partner agency (e.g. Education, Social Work, Health, Police, Voluntary Sector Service, etc) and wish to refer an individual to our service, you can do so by completing a referral form. We deliver a service which is person-led and person-centred, consequently we would ask that prior to making a referral, you speak with the person being referred and obtain consent from them (or a safe parent/guardian for a child under 12 years of age).
